Vital Statistics - Birth, Death, and Marriage Records
Vital Records can be found in the Town Clerk's office where the event occurred or in the town in which the parties resided at the time of event. These events include Birth, Marriage, and Death records that occurred in the Town of Berlin, East Berlin, and Kensington.

Simply click on the Permitium link above and follow the instructions provided on their site. You can choose between a mail delivery option via USPS or pick up option at the Town Clerk's office during normal business hours. Please note this service allows payment by credit or debit card with a convenience fee.
TO REQUEST A VITAL RECORD THROUGH THE MAIL
When requesting by mail please complete the appropriate application below and mail it to our office along with a copy of your photo identification and a check or money order made payable to "Berlin Town Clerk."

BIRTH CERTIFICATES
A copy of your birth certificate can be obtained from the Town Clerk's Office in the town where the birth occurred or in the town in which the mother resided at the time of birth. The Town Clerk's office can also issue certified copies of birth certificates for residents born in any Connecticut town after January 2002.
- Wallet-sized (short form) birth certificate: $15 - please note that this size cannot be used when applying for passports.
- Long-form birth certificate: $20
- Cash or check only (debit or credit cards are not accepted in our office)
You may request a birth certificate by mail by sending a request form, the appropriate fee, and a copy of a current government issued photo ID. Birth records less than 100 years old may be accessed by the following:
- The person whose birth is recorded (if person is 18 or older)
- Spouse
- Parents
- Children (if child is 18 or older)
- Grandparent
- Legal Guardian
- Person authorized by court order
- Attorneys and title examiners representing the person or such person's parent, guardian, child, or surviving spouse
- Agents of a state or federal agency as approved by the Department of Public Health
DEATH CERTIFICATES
A death certificate can be obtained from the Town Clerk's Office in the town where the death occurred or the town where the deceased resided at the time of death (includes Kensington and East Berlin). You may request a death certificate by mail by sending a request form, the fee, and a copy of a current government issued ID. A certified copy of a death certificate is $20. Debit or credit cards are not accepted in our office.
MARRIAGE CERTIFICATES
A marriage certificate can be obtained from the Town Clerk's Office where the marriage occurred or the town or residence at the time of marriage (includes Kensington and East Berlin). You may request a marriage certificate by mail by sending a request form, the fee, and a copy of a current government issued ID. A certified copy of a marriage license is $20. Debit or credit cards are not accepted in our office.
