Patient Satisfaction

Maintaining a high patient satisfaction rate is very important to the Berlin VNA. Beginning in 2010, the Center for Medicare and Medicaid Services (CMS) now requires home care agencies to administer patient satisfaction surveys using the HHCAHPS standardized questionnaire. This is the first mandated patient satisfaction survey for home care and the results from this survey will be reviewed by CMS and publicly reported on Medicare's website beginning in 2012.

You, our customers, are very important to us. The Berlin VNA has partnered with Fazzi Associates, www.fazzi.com, to measure patient satisfaction with the HHCAHPS Survey.

We welcome your comments and suggestions.  We want to know if something is unclear to you regarding our services, whether it is the care you receive or the care you fail to receive. Your primary care nurse or therapist can answer any questions you may have as well as the clinical supervisors. Call the agency 860-828-7030 at any time during your care to discuss your treatment plan.

Become an Active Participant in Your Care!

We want to help you successfully manage your health and live the best life possible. Every day, you make decisions that affect your health, and making these decisions can be confusing and stressful.
 
The Berlin VNA uses "Integrated Patient Centered Care" to help you feel more in control of your health by involving you in more decisions about it. For example, you will help set treatment goals based on what is important to you, and our staff will design a treatment plan to reach your goals.

Integrated Patient Centered Care Components:

Our Role:
  • Work with you and your family on goals that are important to you
  • Help you learn how to care for yourself and make lifestyle changes to stay healthy
  • Provide home visits by nursing, physical therapy, occupational therapy, social service and/or other support staff as needed
  • Provide telephone calls between visits to monitor your progress and answer your questions
  • Assist you in developing your own personal health record to manage your illness
  • Provide educational materials and resources related to your healthcare needs. 
  • Help you manage your medications and develop a medication system that works best for you
  • Support you and help you build confidence in your ability to handle the day-to-day challenges of living with a chronic illness
 
Your Role:
  • Become an informed and active participant in your healthcare
  • Take ownership of your health management and set goals for your care
  • Adopt healthy behaviors/lifestyle
  • Keep all scheduled doctor appointments
  • Be available for your homecare visits and telephone interactions
  • Call the Berlin VNA first with any changes in your condition or questions about your care
  • Use the telemonitoring system each day, if indicated

Choose a Healthy Lifestyle:

  • Take all medications as prescribed
  • Keep an up-to-date list of all your medications
  • Contact your physician when your condition changes
  • Follow dietary recommendations
  • Keep all scheduled doctor appointments
  • Exercise daily
  • Maintain your personal health record
