The Visiting Nurse Team is committed to communicating and collaborating with the client's physician and insurance representatives on the individuals specific home care needs. Visiting Nurses coordinate and manage all the levels of care and needs with a seamless network of comprehensive services. The patient's health care plan and developments are monitored and reassessed, on an ongoing basis, to meet all the patient/family needs.
